This policy explains how we handle personal information collected through this website, in line with the Data Privacy Act of 2012 (Republic Act No. 10173) of the Philippines. By using this website or sending us your information, you acknowledge this policy.
1. Who we are
This website belongs to the clinic of Dr. Jose Antonio dela Fuente, a general surgeon who sees patients at the hospital clinics listed on our Locations page. For questions about your data, contact the clinic secretary of your preferred location, or send us a message through the Contact page.
2. Information we collect
- Information you give us: your name, phone number, email address, the treatment or health concern you tell us about, and your preferred date, time, and clinic location, when you use our contact or appointment forms.
- Text messages: when you tap Book Appointment, your own messaging app opens with a ready-made message addressed to the clinic secretary you chose. The message is sent by you, through your own phone and mobile carrier, and only if you press send.
- Website usage data: if you accept cookies, we use Google Analytics to collect information such as the pages you visit, the buttons you tap, your approximate location, and your device and browser type. This data is used in aggregate.
Please do not send sensitive details such as full medical records, ID numbers, or payment information through the website forms. These are discussed directly with the clinic.
3. Why we collect it
- To respond to your inquiries and appointment requests
- To confirm, schedule, or change your appointment
- To send you appointment-related emails or messages
- To understand how the website is used and improve it
We collect only what is needed for these purposes and process it based on your consent.
4. Who can see your information
Your information is accessible to Dr. Jose Antonio dela Fuente and the clinic staff who handle appointments. To operate the website we use trusted service providers who may process data on our behalf, such as Google (Google Workspace tools for handling form submissions and email, and Google Analytics for usage statistics) and Vercel (website hosting). We do not sell your personal information. We disclose it to others only when you consent, or when required by law.
5. Cookies
We use cookies only for website analytics, and only if you press Accept on the cookie notice. If you press Decline, analytics stay off. You can change your choice at any time with the Cookie Settings link in the footer, and you can also clear or block cookies in your browser settings. Maps on the Locations page are provided by Google and may set their own cookies under Google’s policies.
6. How long we keep it
We keep your information only as long as needed for the purposes above, or as required by law and medical record-keeping rules, and then delete or anonymize it.
7. How we protect it
This website is served over HTTPS, and access to the clinic’s appointment records is limited to authorized staff. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
8. Your rights
Under the Data Privacy Act, you have the right to:
- Be informed about how your data is processed
- Access the personal information we hold about you
- Object to the processing of your data, or withdraw your consent
- Correct inaccurate or outdated information
- Request that your data be suspended, blocked, or deleted, where allowed by law
- Receive a copy of your data in a commonly used format (data portability)
- Claim compensation for damages caused by inaccurate, unlawfully obtained, or unauthorized use of your data
- File a complaint with the National Privacy Commission (privacy.gov.ph)

9. Children
If a patient is a minor, appointment requests should be made by a parent or legal guardian.
10. Changes to this policy
We may update this policy from time to time. The date at the top shows when it was last changed.
